Quotations are hereby requested for Pre - Medical Examinations for the Lukhanji Waste Facilities Project:

(According to the following terms of reference/specification):

Provide medical assessments for 395 construction workers before assuming duties
Referral to secondary health specialist for assessment and report on
Provide advice and recommendations of health risks in the workplace
The medical practitioner must have a current and valid registration with a medical professional body.
3 -5 years working experience in the medical field and having a private practise.
Company that has fully fitted mobile unit with calibration equipment is an added advantage to be able to go to where participants are as per your advert
Post Graduate Diploma or relevant certificate in Occupational Health medicine
Registration with HPCSA
The occupational medical practitioners are needed in the following areas and must have private practises or partner with medical doctors in these areas:
Komani
Wittlesea
NB: Please note that the total number of employees is 395

Suppliers must take particular note of the following:

Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report (If it was never submitted before)
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company (If the MBD 4 Form was never submitted with the Supplier Database forms)
Submit the valid B-BBEE certificate (If it was never submitted before)
Submit a valid original Tax Clearance Certificate (If it was never submitted before)
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
