Quotations are hereby requested for Post-Medical Examination for the Lukhanji Waste Facilities Project

(According to the following terms of reference /specification):

Provide post medical assessment for 301 construction workers for the project
Referral to secondary health specialist for assessment and report on
Provide advice and recommendations of health risk in the work place
Company with mobile services will be an added advantage
The medical practitioner must have a current and valid registration with a medical professional body
3-5 years working experience in the medical field and having a provide practise
Post Graduate Diploma or relevant certificate in Occupational Health Medicine
Proof of registration with HPCSA
The area where medicals will be conducted is Queenstown and Whittlesea

The closing date and time for the submission of quotations is Wednesday, 13 February 2019 at 12HOO.

Suppliers must take particular note of the following:

Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report (If it was never submitted before)
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company (If the MBD 4 Form was never submitted with the Supplier Database forms)
Submit the valid B-BBEE certificate (If it was never submitted before)
Submit a valid original Tax Clearance Certificate (If it was never submitted before)
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
