REQUEST FOR QUOTATION

Quotations are hereby requested of the Supply and delivery of CHDA COVID 19 Social Relief Vouchers for Sakhisizwe Local Municipal area according to the following terms of reference / specifications:

  • The CHDA COVID 19 Social Relief Vouchers are required ONLY for Superstores / Convenience Stores that are situated at Cala in Sakhisizwe Local Municipality;
  • 93 X R 806.45 (Incl. VAT) CHDA COVID 19 Social Relief Vouchers are required;
  • The appointed Superstore / Convenience Store should deliver the vouchers to the CHDA offices situated at number 15 Warner Street in Komani within 5 calendar days after payment thereof;
  • Vouchers to be valid for no longer than 60 days;
  • CHDA should be reimbursed for vouchers that have not been utilised after the 60 days period;
  • Vouchers may not be converted into cash to any beneficiary at the Store.

PLEASE NOTE:

  • The vouchers will be handed over to beneficiaries by the municipality representative and may be utilised anytime during their 60 days life span.

REQUIREMENTS FOR BIDDING:

The copy of the company’s CIPC / CK registration document must be submitted;

The copy of the company’s Shareholding Certificate (if shareholders are not indicated on the CIPC registration document) must be submitted; and

Only quotations that are submitted for Superstores / Convenience Stores that are at Cala in Sakhisizwe Local Municipality will be considered.

STANDARD COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE;

Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report

Service providers must complete & submit the MBD 4 form to declare the interest of the Directors of the Company

Submit the valid copy of the B-BBEE certificate or a Sworn Affidavit for B-BBEE Level of status of contribution

Submit a valid copy of a Tax Clearance Certificate / of a SARS Tax Pin document

If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.

This is an 80/20 preference point’s system quotation
