REQUEST FOR QUOTATION

Quotations are hereby requested for SIM cards, connection and 10 GB data per month for CHDA Board Members for a period of 24 months according to the following specifications / T.O.R.:

• Once-off SIM card supply and registration
• 10 GB data per month, with top-up option available for 24 months
• Total of 15 SIM cards only
• Services must be rendered within 7 days from the date of the official Purchase Order.

Suppliers must take particular note of the following:

• COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE;

• Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report
• Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company
• Submit the valid copy of a B-BBEE certificate / B-BBEE Sworn Affidavit
• Submit a valid copy of a Tax Compliance Pin document
• If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.

Evaluation criteria (Only applicable to quotations equal to/ above R 30 000.00)

• This is an 80/20 preference points system quotation
• Price is allocated up to 80 points
• B-BBEE is allocated up to 20 points
