Quotations are hereby requested for Supply and Delivery of Material: according to the specification:

- 200 LT Transmission oil 54 TXM (2 drums)

  200 LT Engen oil 15w-40 (2 drums)

  Grease oil 20kg X 2

 Dot 4 brake fluid 20 X 2

 Anti-freeze 20 LT X 2

 

 

 

 

The closing date and time for the submission of quotations is Wednesday ,30 August 2023 @ 12H00

Proposals/quotations must be emailed to   scm@chda.org.za

COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE.

  • Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report.
  • Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company.
  • Submit a valid Tax Clearance Certificate /Pin
  • If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.

 

 

Evaluation criteria (Only applicable to quotations equal to/ above R2 000.00)

  • This is an 80/20 preference points system.
  • Price is allocated up to 80 points & 20 points will be allocated as follows:
  • Black ownership (Max. 10 points)
  • 10 points for black owned company, confirmable by proof of CSD registration and certified ID copy(ies) of entity’s director(s)
  • Nil points for no black ownership
  • Locality (MAX. 5 points)
  • 5 points will be allocated to service providers within CHD jurisdiction, confirmable by valid proof of address/lease agreements/rates account statement
  • Nil points outside CHD jurisdiction.
  • Youth ownership (Max. 2.5 points)
  • 5 points, confirmable by proof of CSD registration and certified ID copy(ies) of entity’s director(s)
  • No points if above age of youth
  • Disability (Max. 2.5 points)
  • 5 points for the people living with disabilities, confirmable by certification document from a medical practitioner.
  • Nil points for people with no disabilities.
