OUTCOME - REQUEST FOR QUOTATION:

BIDDERS:

ISIXWIBA CONSULTING WAS AWARDED THE ORDER.

Quotations/ Proposals are hereby requested for the development of basic environmental assessment report for DEA Lukhanji waste buyback project.

Attributes of the EAP

1. Be independent

2. Have expertise in conducting environmental impact assessments or undertaking specialists work as required

3. Possess excellent understanding of guidelines relating to activities here below

4. Comply with these Regulations;

5. Perform the work the work relating to the application in an objective manner

6. Disclose to the proponent or applicant, registered interested and affected parties and the competent authority all material information in the possession of the EAP and where applicable, that
reasonably has or may have the potential of influencing

I. Any decision to be taken with respect to the application by the competent authority in terms of these regulations; or

II. The objectivity of any report, plan or document to be prepared by the EAP or specialist, ij terms of these regulations for submission to the competent authority;

Scope of work

• Liase with Lukhanji LM and CHDA to facilitate community engagement

• Conduct and Environmental Impact Assessment Study(basic assessment)

• Submit a basic Assessment report to CHDA

Time frame

The task should be completed within 14 working days after the appointment of the EAP

The closing date and time for the submission of quotations is Friday, 23 September 2016 at 12h00.

Suppliers must take particular note of the following:

• Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report (If it was never submitted before)

• Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company (If the MBD 4 Form was never submitted with the Supplier Database forms)

• Submit the valid B-BBEE certificate (If it was never submitted before)

• Submit a valid original Tax Clearance Certificate (If it was never submitted before)

• If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.

Evaluation criteria (Only applicable to quotations equal to/ above R 30 000.00)

• This is an 80/20 preference points system quotation

• Price is allocated up to 80 points

• B-BBEE is allocated up to 20 points
