OTHER COMPULSORY REQUIREMENTS:

The above mentioned four DSTV Explora Decoders must be linked to one suitable satellite dish
Suitable market related products guarantees/ warranty must be provided.
Proof of Level 3 Explora Installer Accreditation from Multichoice
Certificate of Compliance from Accredited institution to be issued after installation and setting up of the account
The Compulsory Briefing Session and Site Inspection will be held in the CHDA Offices situated at number 15 Warner Street in Queenstown on THURSDAY, 20 JULY AT 11H00. Accredited service providers are required to ensure that they sign the attendance register.
Quotations of service providers who did not sign the Briefing Session and Site Inspection Attendance Register will not be considered.
The Suitable Service Providers’ quotations must be supported by a clear project implementation plan with time-lines.
The Project implementation plan will be discussed and finalised with the appointed service provider.
Quotations/ proposals must be supported with clear catalogues or pictures of the offered goods.
The appointed Service Provider will also assist with the activation of the decoders’ debit order account for the CHDA.
The project completion will be signed off after the demonstration of the proper functioning of all the supplied and installed goods.

RFQ/P Closing date and time is Wednesday, 26 July 2017 at 12h00.

Suppliers must take particular note of the following:

Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report (If it was never submitted before)
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company (If the MBD 4 Form was never submitted with the Supplier Database forms)
Submit the valid B-BBEE certificate (If it was never submitted before)
Submit a valid original Tax Clearance Certificate (If it was never submitted before)
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
